The problem is once Neeko lost Thief Glove she still couldn't equip other items after the {{item:1058}} I tried to give her {{item:3040}} but her other 2 items slots were marked as black squares and wouldn't allow other items.

Slow champions cannot run away from a fight and cannot chase down opponents (generally speaking, CC situationally changes this).
This concept is what makes champions like Pyke so obnoxious, its very hard to run from him and its easy for him to run away from you.
If mobility is one-sided and mobility is fun, then fun is one-sided. No one wants to be in a situation where, if you have the upper hand, your opponent can just leave and you cant do anything about it, while on the other hand, if your opponent has the upper hand you don't get that option.
Ekko is another example of this, if you have the upper hand he has a dash, movement speed buff, slow/stun and a teleport to run away with. If Ekko has the upper hand, the same tools can be applied to chase you down with the exception of the teleport.

IMO, even if TT is a dud, the idea of a 3v3 is good. Maybe 3v3 just needs a different map.
TT seemed to me to have suffered in reputation as a less competitive SR, which is obviously not something ARAM has to worry about.
I also never have seen streams or vods of TT so I have no clue what its meta even looks like, which is one of the big reasons why I haven't tried it in a long time, I don't want to simply just jump into a TT game the way I would an ARAM game.
IDK what to suggest really but you have my upvote, I would like to see some version of 3v3 stay in the game.

You might be right about some champions, but again, my point was that it wont be balanced it will simply be different and I point out the case of Aatrox as a good example of this. It is also my opinion that Old Aatrox was not one of these champions that needed a rework.

Good list but I don't know about the last two, random drop is random, dragon randomly dropping a completed item that doesn't fit into your comp is still working as intended.
But it definitely feels like there are different drop rates, spatula for example almost never shows up outside of shared draft.

Kalista is a champion I always think about playing more, but then I remember how much I dislike her R. I don't want any part of pulling or not pulling an ally at the wrong time, my ally going afk or in general not being coordinated with me.
These type of effects (those that manipulate allies) are meant for fancy pro plays and solo-queue trolling imo.
To be on topic, I guess what I'm trying to say is, Kalista wont feel like a real champion to me until her R is reworked and no longer manipulates allies.

I think a Item re-roll would be more reasonable.
For example, have a box near the item island, drag and drop an item you want re-rolled into the box at the cost of 2-4 gold and the box will spit out a randomized item.

I'm not accusing you of making it up, I'm only pointing out that it can only be one or the other.
The thing Is you cant play the game like you normally do and get them, you have to figure out which of your champions have a droppable chest and power farm in a game as well as protect your KDA.
The matchmaking in the game will adjust for skill level so the more you lose attempting this the easier the opponents you will encounter I think.
Some Champions are easier than other too. Bot lane in general is difficult because if your team mate is bad its a 1v2 lane, not to mention coordinated dives.
Also I think you posted elsewhere you mainly play aram. There are some Champions that I regularly get S with in that mode, but again, you need to Champions that have a droppable chest so random pick works against you, non-stop 5v5 also works against you since you might rack up a lot of deaths and CS is limited.
I only play aram for chest when I get lazy about it and I'm not capped at 5/5 chest available.

I sometimes get chests within an hour of availability and some times it takes me a couple of days, but 6 months? Your either exaggerating or your not try nearly as hard as you can. Its meant to be a challenge.

To explain a bit better my suggestion for Q, with the old Dark Flight animation, Aatrox would jump straight up and then dive to the target location, my suggestion is to have a initial cast jump a shorter distance to a target location, with the option of a secondary cast, dive to a new target location or to replace the dive with flying you would hold the initial cast, drag and release at a target location resulting in greater distance traveled.
As for the name I'm ok with Xor'taa, although it sounds a bit like sorta.
About the revive, it is definitely a part of the kit that I enjoyed and would like to keep, but as you said, if we could bring back at least part of him, I think this is the that, sadly, we would have to give up.

Instant KO or in this case near instant KO effects are for PvE games and even then its for the player to use against the environment. When its the other way around the player is given counter options, usually items that prevent the effect outright.
No matter how low the chance of such an effect is, it wont be balanced. You either run into it or you don't, or in the example of Phantom, it either hits a target that gets killed shortly after or that target lives long enough to make the effect relatively useless.
For me this effect is up there with hextech as a terrible option in the game. Other notables are stun, silence, shrink, 100% anti-heal and 100% mana burn.

I was thinking about Cass in this earlier, for her ability she could do her Q which then would turn her next 2 auto's into her E which would make her a more reasonable 2-3 gold Imperial. Your suggestion would have to be 4-5 gold IMO.
Zyra would be interesting as a variant of Gankplank, spreading seeds as her mana bar charges with Q as her ability. Maybe a 4 gold Champion Nature/Sorcerer.
Also Maokai for Nature/Brawler and Ivern for Nature/Guardian.

Maybe an unpopular opinion among old Aatrox mains but hear me out.
Old Aatrox cannot come back as a Darkin. That concept was bastardized since the release of Kayn and the lore changed even more with the full rework.
No point in deleting the new Aatrox just as there was no point in deleting the old one.
With some creativity Old Aatrox can be reintroduced as a new champion and given his old look and voice over as a darkin skin.
Old Aatrox could easily be a Void Champion.

Ask yourself this then:
If it is meant to discourage item stacking, why does it target randomly? Why does it cancel items in an area? Why does it cancel more than one item?
If you position to avoid hextech bomb, how do you then also position vs assassins? Or blitzcrank? Or how do you ever use inline buff items?(locket,etc)
What about the interaction zephyr?
What about Champions that need items to be playable? Like melee champions with RFC or double RFC or GA strategies.
Whats is wrong with fighting a death ball? Isnt that what every player aims to build? Isnt it just the end result of the game being played as intended?
If stacking items is such a problem, why are items stackable?

Remember old Fiora's ult got removed because it was unhealthy design? Not much different than Alpha strike which is still in the game or Rek'sai/Kayn ults added since.
My point is you don't get to keep what you like, they will keep changing things for stupid reasons and then outright delete champions when no one or atleast the vocal majority wont play them anymore.

Even if the duration was 1 second I think it completely cancels out locket and zekes.
It may not be auto-win but it is one of those binary effects that just feels awful like silence, stun, shrink, zephyr, phantom, dodge, etc.
